How to prevent discoid eczema from reappearing and flaring up again?

i have just been recently been diagnosed with discoid eczema. I have scars everywhere because of it and it is very hard to prevent myself from scratching when my skin becomes itchy. Is there any way to stop my skin from becoming itchy in the first place? Or are there any methods i can use to stop myself from scratching myself? since i usually scratch at night when i am not conscious of my actions

and is there anyway to lighten the scars caused by discoid eczema?

    Consider a low strength hydrocortisone for flare up. Moisturize, moisturize and moisturize. Avoid potential irritants in skincare products. The National Eczema Association site rates products and publishes a list of ingredients that are best to avoid
